Case Management Services
The RN Case Managers are available to monitor care the client is receiving and coordinate with other healthcare professionals as needed. RN Case Management Features:
Nightingale Homecare's Registered Nurse Case Managers provide a comprehensive health and medication evaluation, addressing the client's physical, emotional and social needs, as well as identifying the personal and homemaker support services needed. The assessment is then used to develop an individualized service plan and provides a guideline for further assessments. The RN Case Manager schedules the assessment visit with the client and other individuals involved in the client's care. The initial home care visit will include: 1) A safety review of the home for any visible hazards or other home maintenance issues. 2) A complete physical assessment, including an evaluation of current medications. 3) A Registered Nurse Case Manager will contact the client's physician to verify medications, treatments and physician orders. 4) The Registered Nurse Case Manager will then follow-up with the client to review these findings and discuss the individualized service plan and schedule further services. With one phone call to Nightingale Homecare, clients and referral sources are connected with an experienced Case Management Coordinator, who will arrange services and coordinate client care. Once service is initiated, a Nightingale Registered Nurse Case Manager is assigned to coordinate the client's care, with careful attention to every detail.
